What Is a Child Travel Consent Form?

A Child Travel Consent Form is a legal document signed by a child’s parent or legal guardians giving permission for their child to travel with another person. The form confirms that the non-travelling parent, or both parents, grant permission for the child’s travel and names the accompanying adult who will be responsible for the child during the trip.

A Parental Consent Form — also called a Minor Travel Consent Form — is checked by border officials, immigration authorities and border protection officers in many countries. What Should the Template Include?

A comprehensive child travel consent form template should contain:

  • The child’s details — full name as shown on the passport, date of birth, passport details and nationality. Where a child has a different surname from the accompanying adult, the birth certificate reference helps.
  • The consenting parents’ details — name, address, phone number, email address and emergency contact numbers. If one parent is consenting alone, note the reason (for example a court order granting sole custody, or a death certificate).
  • The accompanying adult’s details — full name, date of birth, passport details and relationship to the child. If the child is travelling alone as an unaccompanied minor, name the person meeting them at the travel destination.
  • Travel details — destination country, travel dates, itinerary and addresses, plus whether the accompanying adult may make medical decisions on behalf of the child.

Use a child travel consent letter whenever a child travels internationally without both parents.
